It is the responsibility of all staff to ensure that they understand the requirements of timely and accurate Incident Reporting, allowing for the effective control of a health, safety and environmental incident, as well as to trigger the Root Cause Investigation process. Incident Reporting triggers include incidents (or near misses) associated with personal safety, process safety, security, environmental damage, logistics, other breaches of regulatory and compliance standards, quality, asset utilization rates, reliability, and other potentially serious events.
